Authentic source for storage on dispensed medication and other types of data. The Pharmaceutical Care Data Hub is to be considered a complementary data source augmenting the data locally available in the pharmacy.
The information about the dispensing of medicines is uploaded in a standardized manner in a “Pharmaceutical Care Data Hub” (PCDH) managed by the non-profit organization FarmaFlux.
The encrypted data is stored there for a configurable period. At the same time, a reference will be added in a reference register. In this way, the system will always be informed in the event of the availability of new drug-related information supplied by a particular pharmacist for a particular patient. The pharmacist can, under certain conditions, update or delete a previously registered medication supply.
The pharmacist can request the data present in the PCDH from a patient.
The request is authorized (authentication, valid patient consent) via rules:
The patient is identified under the responsibility of the pharmacist. During his delivery supervision, the patient is asked whether his pharmaceutical care information may be consulted. The initiative here lies with the pharmacist who, depending on the context, can decide to appeal to the available information in the PCDH.
During the retrieval of the pharmaceutical health care information, an actual consent is requested on the eHealth platform. In addition, if the patient indicates that a particular pharmacy does not have access to the pharmaceutical healthcare information, no information will be released. If the eHealth platform cannot demonstrate that this patient indicates that the pharmacy in question is not allowed to consult his data and the eHealth platform can demonstrate that the patient has given its consent, all pharmaceutical care registrations will be retrieved from the PCDH.
In an evolution we distinguish different forms of pharmaceutical healthcare information:
The pharmacist will be able to request a patient’s history of drug delivery for a standard period of 1 year. He can also specifically only request the data that has not been processed in his pharmacy (i.e. the data that he does not yet have in his local pharmacy software).
The data itself, which comes from the local pharmacy software or the Pharmaceutical Care Data Hub, is merged into one message in KMEHR format. There is also a visualization component that shows this data in a simple way.
In principle, the obtained history of dispensed medication does not contain any details about the pharmacy that dispensed the previous medication. However, the process allows an authenticated pharmacist with a highly authenticated patient (via eID) to request the details of the dispensing pharmacy. For this type of inquiry, a motivation is also required.
In a specific extension to the medication history, the pharmacist can request the history of the delivered vaccinations for a standard period of 10 years.
The pharmacist will be able to request a patient’s medication scheme for which 3 latest versions reside on the PCDH.
In extension to the use of the PCDH by the public pharmacies, the pharmaceutical medication schemes drawn up by community pharmacists are made available to patients and other health care professionals.
